However, my custom Hawke is not appearing in the Keep, no new name, no custom appearance, no nothing, just the default Hawke. I was told that your custom Hawke can appear in Inquisition.

Depends on the playthrough. I like custom and default for different moods. And actually, fun fact: the siblings, Leandra, and Gamlen's appearances will change depending on which preset model you use to customise from. I think there are like nine different tweaks based on facial structure and skin tone.
